Can also be caused by split keybed, broken by turning the piano on the 
pedal lyre.

This has been the other usual suspect (besides a cheating jack) when I 
have encountered this symptom previously, but no, that's not it either.

This was Terry Farrell's initial guess over on pianotech, and one of the 
two most common causes of this problem, in my experience.  But, in this 
case, that's not it.

You may find that the jack is cheating and if so turn it back in small 
increments until it plays on the hardest blow.

The problem: There were a couple of notes that, when played really hard, 
would not speak at all (no repeated playing of the note involved).  Other 
than that, these notes work just fine.  This was something I had never 
seen before, so it took a few seconds longer than usual to figure out what 
was going wrong.  Any ideas about what it was?
