Neither do I. In fact, I don't think I've ever seen one. I have seen a few
pianos in which the soundboard has distorted and, as a result, the bridge is
no longer square to the strings. Or anything else, for that matter. But this
is a soundboard failure having little to do with the bridge.

I wrote an article in the Journal several years back (mid-90s?) on this,
complete with a couple of drawings illustrating the various stresses on
soundboards, particularly compression-crowned soundboards, that can cause
distortions like this.
