I agree with you about S-Ps being really nicely put together.  This is a tiny problem with a quick fix.
 
Your first guess is the closest.  It happens on most blows above p, and only when the hammer strikes the string.

Hammer Shanks loose in heads/butts
Catchers loose in butts
Left over voicing needles
Key pins hitting the rear of the mortise
 
Hey, someone has to guess the obvious.
 
Okay, never seen it before, eh?  Does it happen on a slow play of the key (when the hammer does not contact the string)?

Okay, as hammer strikes string, click occurs.  Not all notes, but several.

I was merrily tuning away on this very nice S-P 118 upright, when I started to notice a clicking.  I was in a customer's home, so it's a good thing I already knew the fix.  Not an ordinary fix, mind you, and I've never seen this problem in any other piano.

bridle wires
spring noises
loose keyleads
