S&S D with high strings/low action stack

That fits alright. At least if I understand you correctly.  If the pivot 
is too far forward (towards the pianist) then the jack is angled 
backwards (towards the backchecks).  This is consistant with a good 
spread, and the hammer shank being far off the cushion (capstan adjusted 
very high)

Looks to me like you know what you are doing there Andrew.  After 
leveling keys you went for standard keydip, letoff, and drop and let 
blow distance govern aftertouch.  Always a good basis for diagnosing 
problems with an action.

When you have those measurement you will take.... you probably will have 
your answer before you.

    The jack knuckle angle has the pivot of the jack too far forward
    (towards the pianist) so shimming the whippens out makes sense, until
    I actually have to do it, that is... :-(
