Wouldn't the first inch or so, be the actual area of the block, that the 
threaded portion is making contact with?
It would work on grands, that are drilled through, but not uprights.

Somebody clever out there should be able to design and make a two-step
reamer.
The first inch or so would be slightly undersized and non-cutting, to allow
for aligning the reamer, then the second step of the reamer would do the
cutting.

Bruce,

I do it by hand with what I call soft hands. I let the reamer or drill bit
find it's way down the hole. After a few holes you will get the feel of it.
Start in the treble where it is less critical.
