Some pedal rods are threaded on the outside at the top. These have a threaded cap on top which is screwed up and down to adjust the pedal. On this design the nut must be tightened up against the cap in order to lock it in place. Other pedal rods, like Kawai, have a pedal rod that is threaded on the inside. The adjustment is made with a long machine screw cap that screws down into the inside of the pedal rod. On this style, the nut must be tightened down against the pedal rod itself. So the answer to Lim's question is, for Kawai, tighten the nut down against the pedal rod. I hold the rod tightly with one hand, and use a 10mm open end wrench to tighten the nut down. This is plenty tight enough to hold everything in place, and allows the pedal to be adjusted for different pianists most easily. If you want to really lock it in place, you can use vice-grips to grab hold of the pedal rod just above the guide bushing, then tighten the nut down with the 10mm wrench.
