Hi Jon,
Jurgen is your best bet, he is now carrying a good selection
of good quality German castors. You could get lucky and get a new
replacement.
Failing that. plug the old mortice with a maple dowel using epoxy to glue
into position. Build up the outer area of the leg with epoxy to obtain a
snug fit for the ferrule.
Secure the ferrule with screws set in virgin wood, well away from the old
screw holes. Also make sure that the base of the tenon has a flat face to
fit with the bottom of the ferrule cup.
Not the best arrangement for a caster carrying this type of load. If the
piano is not going to be moved much, it will more than suffice.
