Hi, Ben,
   Yes there is such a thing.  I wrote one several years ago, published it
and sold all the copies I had.  You might get one from the folks in SF.  I
sold them multiple copies about three years ago.
   Since that time, I have donated the data file and all rights to the home
office.  What, if anything, they have done with it I do not know.  You might
call or write them a note of inquiry.
