Avery, At UW-Madison we have forms hanging in each room. Replenish the supply every semester. They are printed on color coded paper so we know if the form is from a piano major grand, or an upright. Every practice room has a key that is returned to an operations desk where the forms usually get turned in. Once in a while we get one in our shop door or the facilities manager picks up one from someone he passes in the halls We know about broken strings and sticking keys, and usually service the request within a working day. I think the system works - of course, - I made the forms and put the hooks in the practice rooms. . It's not hard to make your own and custom design what info you need. Let me know if you want to see a copy. The secret to the success of this system is having the form in the room where the problem can be described and written down. I use a staff, instead of all the number systems to identify the note(s) with the problem.
