Greetings Mr Goodale,
       You need to reissue your permit at a cost of $85. reissue fee, plus
the $45. administrative fee.  Tests do not need to be retaken if under three
years old.  File form #FBTS 1032 RE to add a repair classification. CA Gluec
certification will require an accredited course which may or may not exist in
your area,  contact local universities.   Tuning to a nonstandard pitch can
only be done with FBT approval, using form FBT 1066 440 which will require an
explanation for the reasons for the varience.
