I've found that in situations like Richard's, where a client would like some improvement without a major rebuild, it can be helpful to offer to work on an hourly basis. With money being an issue with many of the older beat up pianos, this allow the client some control over the pocket book. The first couple of hours of work make the biggest difference- the piano can be made playable, get the pins holding (CA glue, if needed), then explain that other work can make further improvements. Some of my clients have chosen this route, and they tell me when I make appointments to plan an extra hour (or two) working on the instrument this year. Depending on the time span, I decide what will done with the service time. (voicing, dampers, action, bridge, the list goes on!)
