Well, Off the top of my head:
(1) Tight jack flange pinning.
(2) Tight wippen flange pinning.
(3) No (or insufficient) lost motion (ie capstan cranked too high).
(4) Lousy (fuzzy) buckskin substitute necessitating extra lost motion.
(5) Weak or broken jack spring.
(6) Glue joint between jack flange & wippen broken & jack wanders side =
to side & up & down.
(7) Jack flange pin loose & has wandered out, with pin rubbing against =
neighboring jack flanges.
Well, that's a start isn't it?
