Well, Off the top of my head:
(1) Tight jack flange pinning.
(2) Tight wippen flange pinning.
(3) No (or insufficient) lost motion (ie capstan cranked too high).
(4) Lousy (fuzzy) buckskin substitute necessitating extra lost =
motion.
(5) Weak or broken jack spring.
(6) Glue joint between jack flange & wippen broken & jack wanders =
side=20
to side & up & down.
(7) Jack flange pin loose & has wandered out, with pin rubbing =
against=20
neighboring jack flanges.
Well, that's a start isn't it?
