plaque for piano

Hi All,

I received a fairly uncommon request for me.  I've a client that wants to 
make a dedication to another, in the form of a plaque to be affixed to the 
piano.

Anyone have a recommended source for this/manner of affixing?  We haven't 
settled on how and where and what exactly, so all suggestions are welcome.
