I have sold gift certificates many times.  You all have computers and you can print something up on your letter head or with your logo that looks professional.  It is meant to be opened as a present so it should look good.  I mail mine in an oversized envelope so it will not be folded.  My wife has laminated them in plastic for better presentation too.  As Wim said, it is good to get the money first and do the work later!  I would also specify tuning only... or offer a half day's service.  Even if it a piano you have never seen before, you can sell a slightly bigger job.  Often people are looking for a gift more in this price range too.  You can plan to do some cleaning, hammer reshaping/voicing, and improve the regulation in half a day.  It removes the awkwardness of asking for more money for a pitch raise or something; gifts should not cost the recipient.  Even if the buyer does not choose this option, it makes your normal tuning fee seem like a bargain.
