You may never run into this but when I started working for a store in Dallas (early 1970s) they gave me "statements" to fill out and give to customers.  I had one (commercial) client who would not pay it because it was company policy not to pay statements, only invoices.  There is a difference.

Here are my worksheet and a statement for an appt tomorrow a.m.. I print it at home before I leave, then fill in the blanks when I'm done. They're printed from my home-grown (and not ready for prime time) system.
