Marshall,
     Glad you found the problem. I didn't chip in as the jack under the butt as opposed to resting on the square was my guess also. 
    Gerald has a good tip on getting the spoon out of the way before inserting the screw. 
    One piece of advice is to check the damper timing on any of the notes that had the spoon under the screw head. You may have bent the spoon in doing so. .
 Keep at it --you're doing fine.

  Yes. After 30 plus years in the trade I was halfway through adjusting capstans to return hammers back to the rest rail on a spinet when I noticed the sides had come unglued and caused a condition I attributed to a misadjustment by a previous tech.

     If I had moved the piano a few inches or even just looked more closely it would have been obvious but I rushed in , wasted time and had to repitch the job to the owner . 
      The good news is that as you build experience  these events will happen less often --That's what I'm telling myself anyway.
