<html> John and List, There are 2 things which cause the cable to come out of the pedal slot: 1. The casting has a flaw so that the cable end is not fully engaged into the pedal.  Remove the pedal and turn it over, and clean up the casting with a screwdriver or pointed file. 2. The players are messing around with the pedal, working it up and down very quickly in a way that it was not intended.  The muffler rail flops around, and the cable goes slack, allowing it to pop out. There is a small nylon plug that fits into the pedal just behind the wire that will also help.  These are frequently lost, and you can get another one by contacting me at Kawai.
