Fred, I don't understand the need for question 2 in your survey: "2) Would you describe this institution as a) Prestigious conservatory b) Prominent music school c) Music department of national reputation d) Average music department or other" It seems to me that this would be a subjective evaluation. I think the first question is enough to cover this information without asking for a biased opinion.
