Guidelines addendum

Thanks to all who responded with input and comment on the notion of
creating an appendix giving some "real world" data with regard to
workload. I have incorporated the various suggestions into survey
wording as follows: 
(Please don't "fill in" the survey at this point. I am sending it as a
draft, asking for any further input you might have).

1) Name of your institution.

2) Would you describe this institution as 
a) Prestigious conservatory
b) Prominent music school
c) Music department of national reputation
d) Average music department or other

3) Total number of pianos

(Optional:
Number of grands 
Number of uprights
Number of other (Harpsichords, etc))

4) Total number of full time equivalent technicians (One full time
equivalent equals approximately 2000 hours/year. If you are a contract
tech, please figure your response on that basis. If you work on "per
service" rather than per hour basis, give a reasonable estimate)

5) Your assessment of the overall condition of the pianos at your
institution, and the adequacy of care they receive.
a) Excellent
b) Good
c) Adequate
d) Barely adequate
e) Inadequate

If you have the time and inclination, please answer as many of the
following questions as you can as well:

6) Average age of grand pianos
a) 0 - 20 years
b) 21 - 40 years

responses are in, we'll see what additional measures we want to take in
terms of contacting institutions directly and the like.
