One of the common (meaning regularly occurring) problems I find with lyre and trapwork "problems" is maladjustment. And, surprisingly enough, many technicians go in to troubleshoot a "pedal problem", sit down, articulate the pedal a few times and go "yup, this is a problem". I have done this, too.
