My father had this idea that would be ideal for paralyzed people.  You make
a gadget consisting of a mouthpiece connected to a servo mechanism that
would be able to move the pedal.  

For comfort he could sit in one of those ergonomic computer chairs with his
feet far away. He wouldn´t have to do anything but chew with the correct
intervals.  
Of course if he has TMJ I don´t know what you can do;)
