You don't have to lift the piano. Susan Graham wrote an article on how to 
build a box, on which you can put a jack. It works great. 

I double the price of the truck, and charge a tuning service call to assemble 
and install the truck, which takes about an hour. When you order it, have the 
company send it directly to the church. The boxes in which the truck come are 
heavy. Hopefully the church will have a had cart of some sort on which you 
can carry the boxes.

If you order from Schaff, the caster are already bolted on the truck. They 
also make a universal size truck. It fits on anything from a 5' -- to a 7' 
grand.
