Some foreign object got jammed under the back
regulating button of the rep lever. Rep lever
stays down, jack plays once, then is caught on the
front of the knuckle, since the hammer is unsupported
by the rep lever.

I had one of these once -- a giant paper clip
wandered into a newish Steinway B. It was sporadic.
If you fiddled with the key, the hammer would
sometimes end up balanced on the end of the jack.
