here's a sample contract

There was some messaging recently about tuning contracts for schools, churches, etc. Here's the one I came up with, in case anyone is interested. Feel free to adapt it to your purposes. The lawyer from the school said it seemed fine.

SAMPLE SERVICE CONTRACT

This contract is between the __________ School, (address)
hereinafter referred to as SCHOOL, and ABC TUNING, a piano tuning
and repair service business owned and operated by
___________________, (address) hereinafter referred to as ABC.

The purpose of the contract is to provide regular and profesional
maintenance for SCHOOL's (make) piano, model _____, serial #
_____, hereinafter referred to as PIANO.

ABC will provide a standard A=440 Hz. (plus or minus 3%) equal
temperament tuning for PIANO twice each year, once in the month
of ________ and once in the month of ______. In addition to the
tuning, ABC will provide at no additional charge up to $_____
worth of minor adjustments and repairs as necessary.

SCHOOL will pay $____ for this service, each time it is
performed, a total of $_____ each year. ABC will leave a bill
which SCHOOL will pay within 30 days of the service.

Any extra work needed must first have verbal/written (circle one)
approval by SCHOOL person who signed this contract.

SCHOOL will take care to provide proper care and protection of
PIANO at all times so as to avoid abuse and damage.

A mutually agreeable time for the service appointment will be
arranged by phone between SCHOOL and ABC. SCHOOL will arrange for
a noise free environment for a time period of ___ hours each time
this service is provided. 

This contract will be in force on the date it is signed by both
parties, and will remain in force for a period of 24 months. At
that time a new contract must be signed to continue the service.
