Hello

   The closest yet would be the sostenuto rod, 
but it was not that. Nothing to do with bushings.

   The action worked just fine out of the piano.
Only when placed properly would notes stay down. 
So definitely something to do with stuff going on 
back there. 
 
   Again, not seen this one in 25 years.

   Not a nest, or something fallen into the piano, 
not a loose screw.
