The best way I've found to document my evaluations is to transcribe a written report/checklist from a digital message recorded on the spot and then archived. It's so much more detail-rich and custom than checks on a form looked at later. Recording the information while I'm in front of the piano, and in my own words, is much better for my purposes. $.02 cents' worth...
