Marshall: My experience has been that the mostly likely cause is an excessively hard blow caused the wire to lift out of the slot, as sometimes happens when someone lets us to quickly on the sustain pedal and the lift rod jumps out of the socket. My guess is that it happened the last time someone played the piano right before they left. Patrick C.
