bobbling hammers, again

OK Wim, time to put on the detective hat.....

1. Three notes won't work; same in the piano or on the bench? All together, 
or spread out?
2. Swap some hammer/shanks with a note that works; does the problem travel 
as well?
3. Assuming all flanges equal, no chucking keys?

Isolate, investigate, the answer is there someplace.
