Daniel, 

Besides everything else that has been posted about this topic, you have to consider the time that you spend driving long distances to these appointments. This is time that you could spend productively (i.e. earning money) were you to be working in your shop or running service calls closer to home. You are entitled to be compensated for the extra time that you spend driving to and from these distant appointments. You should build that into the charges for these clients, and use this as part of the explanation clients who question your charges. 

This is similar to what taxi drivers in many jurisdictions are allowed to do on distant trips. They are allowed by their licensing authorities to charge double the meter fare (or some other sort of surcharge, or sometimes flat fares based on double the mileage from downtown to downtown) - to compensate them for the time and mileage they use up to return to their normal working area.
