There are (or, at least, there USED to be) "T" handles available for combination tools. Or, you can make something from a tire iron. Cut off one of the lug tips, grind a slot to receive agraffes, and apply heat shrink tubing around the arm of the iron the slot is cut into to prevent it from slipping off the agraffe. The great fly-wheel effect comes in handy with all of those fines threads to deal with both coming out and going back in. Or you could just chuck that combo tool into a power drill (with a clutch, for installation).
