Would anyone care to share what type of 2+ part paper invoice forms you use for service calls? Do you use an online vendor, local print shop or do you create your own forms? What are the important terms of service to include on the form? Do you put them on the back or bottom of the form? I'd love to avoid duplicate data entry (on the form onsite, then into the computer later) but the advantage of leaving the client a paper copy of the invoice (or quote for future work) seems to trump other methods.
