Instead of filing the little cone shaped nub off of the hammer head remover pliers or shank extractor, try filing it down to a smaller nub.... and sharp, too. Just enough left to dig in. Keeps it from wandering, but doesn't expand the shank inside of the hammer. Not perfect, but sure has improved the tool for me.
